Transition Model for the Region: early years settings to primary schools

Research shows us that young children aren’t yet able to manage transitions on their own. This developmental fact means that it’s crucial for important adults in a child’s life to help establish well-planned and predictable transitions, to support healthy emotional development. This includes transitions between the home and the setting/school, and between settings and schools as organisations.
Through the CTM EY transformation programme, and in collaboration with The Learning Partnership (TLP), staff across the CTM region have developed a regional transition model and a culture of information sharing between early years settings and primary schools. It is hoped that continuing to promote an agreed transition model will support the developmental progress of all children transitioning into statutory education.
This section includes a report from TLP detailing the results of piloting work and a document summarising the agreed regional transition model. In addition, a suite of resources is included, to be used by local authority partners as a “Train the Trainer” approach to support staff development and embedding of transition practices in the early years.
